Sample Introductory Language

Many find it helpful to have language about how to frame the survey for the seniors to be interviewed. The following sample language is one suggestion:

“Hello, Mr./Mrs. Name.  Here at Organization Name helping people maintain their health and well-being is very important to us.  To get a better understanding of what the top health issues are and what services are most needed, we are asking all our clients questions about their health.  Your answers will help us better serve you and the other seniors in our community. This should take about 15 minutes.  Is this a good time?”

Note that you may have better success if the introduction does not sound overly formal, so those administering the survey should try to speak naturally and use language that is comfortable for them.
